Direct Access is forbidden
error messare are displayed when anyone try to use an internal PHP page that was not made to be directly referenced in a url.
The problem occurs on the whole platform or only for some courses?
For all question bank operation ? For all operation involving tests ?
Have you checked the filesystem and the integrity of PHP files ?
What docebo version are you using ?

Thanks for your reply.
The problem occurs whenever the gradebook is clicked in the teacher area of a course. Later it was learned that at any time a teacher/admin goes to "Add question" to any test, the error message comes up.
The platform is running the last public docebo version 4.0.5
We have checked the files system and there are no errors with is and it is as healthy as possible.
I do not currently have a way to check the integrity of the PHP files; please advise.

Correction: The problem does NOT occur every time the gradebook is selected, only for certain courses. Does appear to occur every time the "Add Question" button is selected.

It seems be a server , filesystem or data issue.
I suggest you a professional service to inspect your installation and data (db) to found the issue and the needed correction.
If you are interested send me a PM.

The problem appears to have been caused by a bad php file.(class.question.php)
When I went through and compared current to original files, I was able to make the correction.
Once replaced, system functions as normal.
Thank you for your support and direction, I will call upon you in the future when things are beyond me.
thanks again.
